Immunization information for students entering Kindergarten and 7th Grade

Under a new Tennessee immunization requirement, children entering 7th grade in 2010 will be required to have a Tdap (Tetanus, Diptheria & Pertussis) and Varicella (Chicken Pox) booster.

The Rural Health Services Consortium Mobile Medical Clinic will be available on the following dates
to administer required immunizations to students entering kindergarten and 7th Grade:

• Friday, April 30th: Surgoinsville Elementary and Middle 1-4 pm
• Friday, April 30th: Bulls Gap School  1-4 pm
• Friday, April 30th: Clinch School 1-4 pm
• Friday May 7th: Surgoinsville Elementary and Middle 1-4 pm
• Friday, May 7th: Bulls Gap School 1-4 pm
• Friday, May 7th: Clinch School 1-4 pm
• Wednesday, August 4th: Mooresburg Elementary 10am-2pm
• Thursday, August 5th: Joseph Rogers Primary 10am-4pm
• Friday, August 6th: Church Hill Elementary 10am-4pm
  
  Any student entering kindergarten or 7th Grade may visit the Mobile Medical Clinic on these days to receive immunizations.  A parent/guardian must be present and also bring their child’s shot record in order to receive immunizations.
